Case management is available to all plan members who need assistance with coordinating health care services and\/or accessing resources. Registered nurses, pharmacists and social workers, referred to as Care Managers, assist members with needs spanning behavioral services and the medical community. If you are facing a serious illness or medical condition, then Case Management may be right for you.<\/p>\n\n\n\n<p><strong>Case Management Services<\/strong><br>General Case Management and disease specific case management programs are available. The goals of these services are:<\/p>\n\n\n\n<ol class=\"wp-block-list\">\n<li>To increase member care through coordination of services between the primary care provider, specialist and other health team members;<\/li>\n\n\n\n<li>To provide ongoing education and prevention resources to our members; and<\/li>\n\n\n\n<li>To improve the member care experience.<\/li>\n<\/ol>\n\n\n\n<p>Examples of diagnoses and\/or conditions that we frequently assist with include:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Asthma<\/li>\n\n\n\n<li>Chronic Obstructive Pulmonary Disease (COPD)<\/li>\n\n\n\n<li>Depression<\/li>\n\n\n\n<li>Cerebral Vascular Accident (CVA)\/Stroke<\/li>\n\n\n\n<li>Diabetes<\/li>\n\n\n\n<li>Heart Disease<\/li>\n\n\n\n<li>Medication Management<\/li>\n\n\n\n<li>Substance Misuse<\/li>\n<\/ul>\n\n\n\n<p>Complex Case Management is another program available to our members. If you have complex health needs, we can help you manage your plan of care. Examples of when you may need to enroll in case management are below, but keep in mind these are not all of the reasons:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>You are going through a transplant<\/li>\n\n\n\n<li>You have had multiple hospitalizations<\/li>\n\n\n\n<li>You have been told you are pregnant and are at high risk<\/li>\n\n\n\n<li>You have a severe and persistent mental illness<\/li>\n<\/ul>\n\n\n\n<p><strong>Contacting Case Management<\/strong><br>If you have any questions about our Population Health Management program or would like to make a case management referral for yourself or a loved one, please call Alliant\u2019s Member Care Line at: (800) 865-5922. All Population Health Management programs are provided by Alliant at no cost.<\/p>\n\n\n\n<p><strong>Preventive Health<\/strong><br>Alliant Health Plans has also established preventive health guidelines to improve health care quality, reduce unnecessary variation in care, assist providers in guiding and educating patients, providing a basis for wellness programs, and to improve members\u2019 health. These guidelines will help guide wellness interventions,<br>educate providers, and encourage self-management lifestyle changes for Members.&nbsp;<a href=\"http:\/\/webapp01\/ahp-old\/preventive-guidelines\">The preventive health guideline is available at AlliantPlans.com<\/a><\/p>\n\n\n\n<p>For the management of health needs, Alliant Health Plans adopted the United States Preventive Services Task Force (USPSTF) A and B recommendations, healthcare.gov, and the Centers for Disease Control (CDC) recommendations for the following categories:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>\u00a0Pregnant Women<\/li>\n\n\n\n<li>Children 0-24 months<\/li>\n\n\n\n<li>Children 2-19 years<\/li>\n\n\n\n<li>Adults 20-64 years<\/li>\n\n\n\n<li>Adults 65 years and older<\/li>\n<\/ul>\n<\/div><\/div>\n\n<\/div><\/div>","protected":false},"excerpt":{"rendered":"<p>WE\u2019RE HERE FOR YOU!
